This program is open 365 days a year to all Pantry members and provides benefits screenings by case managers to check for sufficient family income and resources.  In addition to screening, 365 YCP staff assist with acquiring benefits and entitlements such as Supplemental Nutrition Assistance Program (SNAP) or Social Security Supplemental Income or make employment referrals; and provide assistance with accessing the other appropriate resources to prevent families from being evicted from their homes.  Through unique collaborations with our social service providers, 365 YCP staff directly process SNAP and Medicaid applications for families on-site.  In addition, 365 YCP provides ready-to-eat brown bag meals on days that the Hot Meal Program does not provide meals.  In FY 11, YCP and its on-site providers accessed $3,475,477 in new benefits for Pantry members, more than double the amount accessed in FY 10.


YCP Program Goals & Outcomes:  365 YCP Case Management


FY11 Goal:  Insure that at least 25% of YCP clients receive services other than food, such as Food Stamps, Public Assistance, and SSI/SSD, resulting in at least $1.1 million in new benefits.  Ensure that 100% of new pantry participants are screened for social services.

Outcome:  42% of clients, a total of 2,937 households, received services other than food.  In addition, 100% of new households, a total of 3,390 were screened for services.  With the help of onsite partners, new benefits of $3,475,477 were accessed for pantry clients.

New Goal for FY12:  Secure at least $2.5 million in new benefits for participants.  Ensure 220 households receive legal counseling services from on-site provider.  Ensure at least 270 households receive multiple benefits.
